(7) Data refer to all treatments up to 2002. For 2003 data refers to first treatments. Heroin includes all opioids. For 2004 data was not available therefore not included in the analysis.

(8) Data for 2003 include 24.4 % of main drug (heroin + cocaine). In former years this situation was coded as 'cocaine' in table 4.

(9) Data refer to all treatments up to 2002 and to new treatments from 2003.
