(5) Data refer to all treatments up to 2002. For 2003 data refers to first treatments. Heroin includes all opioids. For 2004 data was not available therefore not included in the analysis.

(6) Data refer to all treatments up to 2002 and to new treatments from 2003. Before 2003 data only refer to amphetamines.
